All the benefits that are covered under Medicare, MassHealth and Medicare Part D plans are rolled into one package under SWH. Instead of three health cards, you will have one SWH card that covers everything.
As a member of SWH, our nurses, social workers from your community elder agency and other staff will work with you, your caregiver(s) and your doctor to design an individual care plan that combines your medical services with social and community support services to keep you healthy, secure and independent.
There are no co-payments or premiums for any services, including your medications, if you meet MassHealth Standard income guidelines.
With SWH, you get:
- All your Medicare, MassHealth and Medicare Part D benefits in one plan.
- Care management and a care team to help plan your care. This includes a social worker from your local elder agency, a SWH Nurse Care Manager, a SWH Community Support Coordinator and others if you need them.
- Individual care plans.
- Support for your family and caregivers.
- Other benefits including exercise classes, chronic disease management support, eyeglasses, etc.
This plan is available to anyone aged 65 or older who is enrolled in MassHealth Standard.
