Our Care Model
As a health care plan for those who are eligible for MassHealth Standard and who may also have Medicare, we’re definitely not your typical health care plan.
SWH uses an approach to coordinating care that includes our members and their caregivers in decision making related to their care. We offer an array of services based on our member’s medical, behavioral, long-term care, spiritual and social needs.
SWH helps to coordinate and integrate care for each member. Your care team includes your Primary Care Physicians (PCP), SWH Nurse Care Managers, a social worker from your local Aging Services Access Points (ASAPs), as well as behavioral health and medication experts and any other type of provider recommended by your care team.
