SWH is dedicated to providing our members with the best care possible and keeping them healthy and independent.

SWH Nurse Care Managers are assigned to our Primary Care Physicians and Skilled Nursing Facilities (SNFs). These nurses help plan, organize and arrange services for our members in cooperation with the doctor, the local elder services agency and others involved in our members’ care. These Nurse Care Managers are available by phone 24/7 both to our members and providers in order to facilitate treatment.

Care is coordinated across all levels from medical and behavioral health to long-term care and psychosocial needs.
