About the Plans
Senior Whole Health of New York HMO SNP (SWH) is a flexible health care plan that combines traditional health care services with social support services that will keep you healthy, secure and independent. SWH provides a coordinated plan of services to our members whether they live at their own home, the home of a responsible adult, or an adult care facility (other than a shelter for adults).
Our Programs
SWH offers two programs to ensure we can service your individual needs and the specific needs of other dual-eligible persons:
- SWH (Medicare and Medicaid Advantage) provides health care benefits and coordinated care for anyone aged 65 or older and who has NYS Medicaid and Medicare and live in the SWH service area.
- SWH Plus (Medicare and Medicaid Advantage Plus) provides long term care benefits to people aged 65 years or older and chronically ill who need medical and social support services. This plan is offered to maintain people in their homes who are eligible for NYS Medicaid and Medicare.
Annual Contract Renewal:
Contracts between plans and Medicare are renewed annually. Plans and/or Medicare may choose not to renew a contract resulting in termination of the plan and termination of beneficiary’s enrollment in the plan. Plans may also choose to reduce their service area and no longer offer services in the area where the beneficiary resides. If this happens, all members will receive written notice at least 60 days prior to termination of their enrollment, including information on other health plan options and how to contact and enroll in another plan.
