Eligibility

Participation in any of SWH’s programs is voluntary. To be eligible, individuals must be enrolled in Medicare Parts A and B and Medicaid.

Additionally, they must:

  1. Be aged 65 and older;
  2. Reside in the Senior Whole Health of New York HMO SNP (SWH) service area;
  3. Individuals with End Stage Renal Disease generally are not eligible to enroll in Senior Whole Health of New York HMO SNP (SWH) unless you are members of our organization and have been since your dialysis began.

SWH members are also eligible to enroll in our Medicaid Advantage Plus program, if they:

  1. Have a chronic illness or disability that makes them eligible for services usually provided in a nursing home;
  2. Are able to stay safely at home at the time of joining our plan; and
  3. Are expected to need one or more of the following services for at least 120 days from the date of joining our plan:
  1. Nursing services in the home
  2. Therapies in the home
  3. Home health aide services
  4. Personal care services in the home
  5. Adult day health care, or
  6. Social day care if used instead of in-home personal care services.
