How to Enroll
Senior Whole Health of New York HMO SNP (SWH) is a new health care plan for people who qualify for both NYS Medicaid and Medicare and who live in the SWH service area. SWH is a Medicare Special Needs Program that is committed to providing health care to New York State’s dual-eligible population.
Upon your enrollment with SWH, you will notice that we are definitely not your typical health care plan. Our members, our members’ families and their primary care doctors prefer our plan. That’s because SWH:
- Becomes your one-stop contact to a full range of health care support services, including nurses and social workers, other professionals and community agencies
- Coordinates all your care from physician visits to health club/gym memberships
- Provides preventative services such as flu shots and annual physicals
- Treats you with the loyal, loving care and respect you deserve
- Is available to answer your questions 24-hours-a-day
Enrolling in any of the SWH programs is simple.
Simply call us. We are available to assist you in completing your application at any time. We actually encourage you to take advantage of this opportunity to sit down in person and complete the application.
