Drug Benefits

As a member of Senior Whole Health of New York HMO SNP (SWH) you will still receive your Medicare Part D benefits.

This Drug Benefits section of the Web site will provide you with a more detailed overview of SWH’s Part D drug benefits plan, as well as information and tools you will need as a member of SWH who qualifies for Part D.

You pay a $0 annual deductible.
You pay a $0 premium because of your eligibility for Medicaid coverage.

Senior Whole Health of NY is voluntary health plan with Medicare Advantage and NYS
Medicaid contracts. This plan is available to anyone aged 65 or older and who
has both Medical Assistance from the State and Medicare. The benefit information
provided herein is a brief summary, not a comprehensive description of benefits. For
more information contact the plan. Benefits, formulary, pharmacy network, premium
and/or co-payments may change on an annual basis. As a member of our plan, your
Part B premium is covered by NYS Medicaid. Limitations, co-payments, and restrictions
may apply. Eligible beneficiaries may enroll in our plan at any time of year. Plan
members must use network pharmacies to access their prescription drug benefit, except
under non-routine circumstances. Quantity limitations and restrictions may
apply. Co-payments may vary based on the level of Extra Help that you receive. Please
contact Member Services for further details. You may be able to get Extra Help to
pay for your prescription drug premiums and costs. To see if you qualify
for extra help, call:

a day/7 days a week;

The Social Security Office at 1-800-772-1213 between 7 a.m. and 7 p.m., Monday through

Your State Medicaid Office.

You must have Medicare Part A and Part B to enroll.
