Member Eligibility
Participation in any of SWH’s programs is voluntary. To be eligible to enroll in any of SWH’s programs, individuals must:
- Have full Medicaid coverage
- Have evidence of Medicare Part A & B coverage; or be enrolled in Medicare Part C
- Reside in the service area of the plan, which includes Albany, Columbia, Dutchess, Greene, Montgomery, Orange, Saratoga, Schenectady, Rensselaer, Ulster, Warren and Washington counties, Bronx, Kings, New York, and Queens Counties, NY
- Be 65 years old or older
- Enroll in the same plan’s Medicare Advantage Product
