Medication Therapy Management Program

What is a comprehensive medication review?
As a member of Senior Whole Health (HMO SNP) and Senior Whole Health NHC, you can have a pharmacist sit down and review all your medications with you. This program is not considered a benefit and you are encouraged but not required to participate. You are able to review your medications face-to-face with a pharmacist. Much like you have a physical, this is a medication check-up. These pharmacists do much more than fill prescriptions. They are there to help you get the best results from your medication. They can answer your questions and work with your doctor.

Who's eligible?
All Senior Whole Health members enrolled in one of our HMO SNP plans are eligible to participate at no cost. If you meet all the following criteria, you will be sent an "offer" letter to participate in the program: (1) you are being treated for at least three chronic conditions*; (2) you are taking at least five long term medications; and (3) the annual cost of your medications (not what you pay) will be greater than or equal to $3,967.

*Qualifying chronic conditions include: Osteoarthritis, rheumatoid arthritis, chronic heart failure, diabetes, high cholesterol, high blood pressure, depression, asthma, chronic pulmonary disease, and COPD.

What can you expect?
This service requires an appointment. The pharmacist will have a list of everything you have filled but you should bring all your medicines with you, especially over-the-counter and herbals. The pharmacist will look at how all the medications work together and may make suggestions to make it easier to take your medications. The appointment should last about 20 minutes. When you finish your appointment, you will leave with a written list of medications and instructions that you can share with your doctor or other caregivers. If you are unable to travel to a pharmacy, please contact member services for transportation assistance to a pharmacy or to schedule a telephonic medication review with a pharmacist.

Getting started is easy.
The pharmacist that already fills your prescriptions may already provide this service. You can ask your pharmacist if he or she is an "Outcomes" pharmacist. "Outcomes" is the company Senior Whole Health works with to provide you this benefit. If your pharmacist does not participate or you would like to use another pharmacist, call our customer service number Monday through Friday, from 8 A.M. to 8 P.M. at 1-888-794-7268 (TTY/TDD 711) for additional information. If you choose another pharmacy for this service, you can continue to fill your prescriptions at the pharmacy you are currently using.
