Who can join Senior Whole Health of New York's Medicaid Managed Long Term Care (MLTC) Plan?

To join the Senior Whole Health of New York Managed Long Term Care Program, you must be:
  • At least 21 years of age
  • A resident of Bronx, Kings, New York, Queens, or Westchester County
  • Is determined eligible for Managed Long Term Care by Senior Whole Health of New York using an eligibility assessment tool designated by the State Department of Health
  • Capable at time of enrollment of returning or remaining in your home and community without jeopardy to your health and safety
  • Assessed as requiring at least one of the following long term care services and care management for more than 120 days from the effective date of enrollment:
    • Nursing services in the home
    • Therapies in the home
    • Health aide services in the home
    • Personal care services in the home
    • Adult day health care
    • Private duty nursing
    • Consumer Directed Personal Assistance Services
  • Determined by the New York City Human Resource Administration (HRA) Medicaid Office or your Local Department of Social Services (LDSS) Office to be eligible for benefits under the Medicaid program

Our service area includes Bronx, Kings, New York, Queens and Westchester counties.

What is the MLTC Plan?

The MLTC Plan provides benefits and service to adults 21 or older who are Medicaid eligible and need long term support and assistance with day-to-day activities but choose to remain living at home.

Does membership in the MLTC Plan change my Medicare coverage?

Membership in Senior Whole Health of New York's MLTC Plan does not change your coverage for important medical benefits such as ambulance, doctor visits, emergency room care, hospitalizations and prescription drug coverage. If you have Medicare you will continue to be covered by Medicare for these services.

Does membership in the MLTC plan change how I use my Medicaid card?

If you use your Medicaid card when receiving services for important medical benefits you can continue to use your card.

When is the open enrollment period?

Eligible individuals may enroll at anytime of the year. There is no open enrollment period.
