The SWH Whole Health FIDA plan provides benefits and services to adults 21 or older who have both Medicare and Medicaid in New York State and require community or facility based long-term services and supports (LTSS). Participants must live in the SWH Whole Health service area which includes Bronx, Kings, New York or Queens counties.

How does the plan work?
Our team works with you and your Doctors, Specialists, Home Health Aide, home and community or facility based long term care services — along with family members or caregivers — to help you get the Long-term services and supports (LTSS) you need, where you live. SWH Whole Health FIDA Plan is here to help you and your caregiver. We are part of your team.
SWH Whole Health FIDA Plan is a managed care plan that combines and coordinates all of your services. With SWH Whole Health FIDA plan you use only one card to get all the care that you need. FIDA is like combining your Medicare Advantage plan/Original Medicare, Medicare Part D and Medicaid Prescription Drug Plan, Medicaid Managed Long Term Care Plan, and your regular Medicaid card into one plan. And, we help coordinate all your services with you and for you.
All of these services are now covered by SWH Whole Health FIDA Plan. Your SWH Whole Health FIDA Plan Care Team will work with you to coordinate all of your care with you and for you.
